Dear Parent or Guardian of a Student in the Lakewood City Schools,
The Lakewood City School District and Cleveland Clinic Children’s are discussing the possibility of opening a School-Based Health Center within the school district to provide medical health services for students in Lakewood City Schools. Although we will “pilot” this program at Harrison Elementary School and Garfield Middle School, your cooperation is important for future planning and expansion.
We want to learn more about the specific health needs of students and their families. In order to help us plan for the School-Based Health Center, we would like to ask you a few questions. Your answers are completely confidential. You do not need to put your name anywhere on this form. Please complete the survey at the link below by Tuesday, Feb. 24. Thank you for your help.

The Subsidium report prepared for the Hospital board included school based and mobile health units as a possible replacement for services provided by Lakewood Hospital. My guess is funding for the program will come from money due under the Hospitals agreement with the Clinic.
ALTHOUGH
Since the Hospital is a public institution run by a board that is supposed to serve the interests of the people of Lakewood so you would think that we wouldn't have to
wonder and suppose and guess and read between the lines to find out what's going on.

And of course Google will be their with support for all of the documents! Store them right
next to all the student common core info!
