Can you recommend a doctor for a family member ?

A family member is looking for someone to recommend a doctor, internal medicine, who has these qualities:
1) Is not so busy as to not return phone calls, since who calls a doctor if it's not important to them?
2) Looks out for his/her patient, knows the patient's name, is interested in helping the patient get well and be well rather than taking the "clinical" road where a staff of doctors take care of x amount of people all waiting in the waiting room to see "someone".
3) Has some years of experience in private practice.
4. Has his/her office in Lakewood.
I think all this describes an "old school" doctor, much like many small towns have.
If you know of such a doctor, please either post their name or message me, and I'll relay the info.
