KINDERGARTEN REGISTRATION
Kindergarten registration will take place in all Lakewood Elementary Schools on Wednesday, February 15, 2006, from 2:00 to 7:00 PM. Children are to be registered at their neighborhood school. Parents may register a child who will turn five years of age on or before September 30, 2006.
The following records should accompany registration materials:
 child’s birth certificate (U.S. citizens) or Passport and/or proper immigration forms
 child’s social security card
 child’s immunization record, the doctor’s name and phone number
 proof of residency
• current gas or electric utility bill with name and Lakewood address of parent or legal guardian, or
• fully executed rental or lease agreement, or
• purchase agreement, or
• NOTARIZED Residency Affidavit
 official documentation of custody (if child does not live with BOTH parents)
 emergency phone numbers:
• father’s business phone number
• mother’s business phone number
• name and phone number of a neighbor or relative
Children enrolled in Project 1-2-3 or Lakewood’s special education preschool programs are already enrolled for kindergarten and do not need to re-register. A kindergarten packet will also be sent to these families.
If you have friends or neighbors who have a child eligible for kindergarten in August but do not currently have a child in the Lakewood Schools, please share this information with them. They should call 529-4203 to request a kindergarten registration packet.
If you currently have a child/children in a Lakewood elementary school, please do not call for a registration packet. You will automatically receive a packet. If you have any questions regarding registration, please call your neighborhood school. Kindergarten packets with registration forms and information will be mailed in January.
