Middle School & High School Students are invited to get their sports physicals for the
2013-14 school year
Physicals for BOYS
Date: TUESDAY, May 21st
Place: Lakewood High School East Gym
Time: 3:00-6:30 p.m.
Physicals for GIRLS
Date: THURSDAY, May 23rd
Place: Lakewood High School East Gym
Time: 2:30-6:30 p.m.
You must have a sports physical on file with the athletic department in order to try out for any sport during the next school year.
Sports physicals are valid for one (1) year from the date of the physical.
This sports physical would cover the fall, winter and spring sports season.
Attention: Parents/Guardians
• Please fill out all pages & sign where necessary before the day of the physical.
• Give all the forms to the doctor at the time of the physical.
• The doctor will fill out pages 3 & 4 of the OHSAA Physical Form.
(Please make sure the student’s name and date of birth is on the physical exam form)
